In keeping with the CDC Guidelines in regards to COVID-19, please read the below very carefully.

Sports Physicals

Please complete or update your Family ID account.  Student's Family ID Accounts need to be updated with any new information for the RCSD Nurses to review.

Student athletes can participate in the Winter 2020 sports season if they have previously had a health examination for school attendance in the 2018-2019 or 2019-2020 school years and provide a complete Rome City School District Athlete Health History form (see link on this page), signed by a parent/guardian within 30 days of the start of the season.

Student athletes who have NOT previously had a health examination must still obtain a health examination prior to participating in the Winter 2020 sports season, in which two forms are required:

1 - Rome City School District Athlete Health History form

2 - NYS Health Examination Form

For your convenience, after the form(s) are completed, they can be uploaded to your Family ID Account.  Printed forms will still be accepted by the Nurse at your school location if you are unable to upload.

Sport Physicals:  

If your child needs a sport physical or a maturity test please set up an appointment with your Health Care Provider prior to January 4th 2021

2. Boys Swim/Dive 

NOTE: This is for anyone who has not had a sports physical in the last two years 2018-2019 or 2019-2020.   An updated Health History Form will need to be completed and turned into the nurses office at RFA and Strough.   The form can be downloaded and found on the RCSD website under athletics.    

Please Note: There will be no MODIFIED sports this Winter Season.

We will only be having Varsity and Junior Varsity sports this 2020 Winter Season.  The HIGH RISK SPORTS AS DEFINED BY THE STATE: are only tentatively scheduled for a start date of January 4th 2021.   Please note the Governor must give the State the green light to start on this date.

Indoor Track will not be conducted due to lack of facilities for this program.
